Your First Appointment

 

First Appointment?

To expedite your first appointment please fill out the form listed below before your treatment at DPT. You can print it out and bring to your appointment, or fill it out at the start of your appointment. Please arrive 10 minutes early to your scheduled appointment if you will be filling out form in clinic.

What will your first appointment consist of?

The first appointment at DPT begins with a thorough discussion (subjective exam) about your history and goals. Secondly, DPT will perform a biomechanical exam and functional assessment of your body. This will help direct a specific treatment plan. The last third of your appointment will be treatment which will start you on progress toward your goals.

 

What should I wear at my first appointment?

Please bring or wear comfortable clothing suitable for exercise.

 

Insurance/Rates

 

Are services covered by insurance?

Insurance Coverage: DeMarco Physical Therapy (DPT) is not a participating provider with any insurance plans. Why is this? The answer is quite simple – DPT would not be able to provide the high level of care and individualized treatment that DPT does at rates insurance companies are currently willing to pay for physical therapy treatment.

 

Can I submit my Bills to My Insurance for Reimbursement?

The answer to this in most cases is yes. Claim forms are available on your insurance company’s website. You will need to fill out claim forms and send it in with the receipt from your physical therapy. The amount you will be reimbursed is dependent on your insurance company. If you call them be sure ask about “out of network physical therapy” coverage.

 

What is the charge per Physical Therapy Session?

The rate for all treatments is $140 per hour session. The payment is required at the start of your session.

 

How do I pay for Physical Therapy Sessions?

DPT accepts cash, check, and credit card. Flexible Savings Account (FSAs) and Healthcare Spending Accounts (HSAs) routinely provide reimbursement for physical therapy services.

 

What if I need to cancel my appointment?

If you must cancel an appointment, DPT requires that you give 24-hour notice by phone 414-416-6747 prior to your scheduled appointment time. There will be a $60 cancellation fee if DPT is not given 24-hour notification.

If you do not come for an appointment, the charge will be the cost of the appointment, $110.

 

Do I need a Doctor’s prescription for physical therapy?

No, physical therapy does not require a doctor’s prescription to in the state of Wisconsin. Some insurance companies require a prescription for payment, so check with your individual insurance company.
